General Purpose
The primary purpose of this job position is to provide each assigned resident with routine daily nursing care and services in accordance with the residents' assessment and care plan, as directed by supervisors.
Essential Duties
- Identify residents using wristband or photo card before administering treatments or serving meals.
- Use authorized abbreviations when recording information.
- Report all changes in residents' conditions to Nurse Supervisor or Charge Nurse promptly.
- Record entries on flow sheets, notes, charts descriptively.
- Report all accidents and incidents during shifts.
- Maintain confidentiality of residents' protected health information and report violations.
- Perform only trained nursing care procedures.
- Prepare residents' rooms and greet residents appropriately.
- Assist residents with personal possessions and transport as needed.
- Create a warm, calm, and positive environment.
- Meet regularly with nursing personnel to identify and correct problems and improve services.
- Report complaints and grievances made by residents.
- Measure and record vital signs; weigh and measure residents as instructed.
- Ensure frequent checking of residents who cannot call for help.
- Assist residents with food arrangements and record food/fluid intake.
- Perform after meal care and assist with daily functions such as dental care, bathing, dressing.
- Assist with lifting, turning, moving, and positioning residents.
- Observe and report skin conditions and injuries.
- Report any resident leaving/missing from the facility immediately.
- Use only trained equipment and report defective equipment immediately.
- Report all allegations of resident abuse or misappropriation.
- Honor residents' refusal of treatment and report such requests.
Supervisory Requirements
This position has no supervisory responsibilities.
Qualification
Education and/or Experience: Minimum 10th grade education required.
Language Skills: Ability to read, analyze, interpret business periodicals, professional journals, and regulations; write reports and correspondence; effectively communicate with managers and employees.
Mathematical Skills: Apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
Reasoning Ability: Solve practical problems with limited standardization; interpret instructions in various forms.
Certificates, Licenses, Registrations: Must be a licensed Certified Nursing Assistant per state laws; CPR preferred.
Physical Demands
Must occasionally lift/move up to 25 pounds. Regularly required to sit, stand, walk, talk, read, or hear. Frequent use of office-related equipment. May assist in evacuation during emergencies.
Work Environment
Noise level usually low to moderate. Reasonable accommodations available as needed.
