Home Care Aides are responsible for providing personal care and designated health related services in compliance with policies and procedures, applicable health care standards, and governmental laws and regulations to patients.
Responsibilities
- Assists client in activities of daily living.
- Assists client with medications, which are ordinarily self-administered, as required by the assigned case and allowed by state regulations.
- Assists in the implementation of a realistic, goal-oriented Plan of Care.
- Follows verbal and written directions provided by the RN or registered therapist, dependent upon the type of assignment.
- Maintains a clean and safe environment and performs light housekeeping assignments.
- Maintains and submits documentation as required. Records observations and completed tasks.
- Observes, records, and reports significant changes in the status of client and/or family to supervisor.
- Other duties as assigned to assist with overall operations of the agency.
- Participates in case conferences as requested.
- Participates in patient case conference, in-services, and staff meetings as requested.
- Prepares light meals as needed per patient request/needs.
- Provides hands-on personal care services to clients.
- Provides personal care, rehabilitative, supportive services to home health clients.
Requirements
- High School (Required)
- Automobile Insurance - Sarah Bush Lincoln
- BLS or CPR certification within 90 days of hire - Sarah Bush Lincoln
- CNA or completion of 1st semester of RN Program - Sarah Bush Lincoln
- Driver's License - State of Illinois
Compensation
Estimated Compensation Range: $18.68 - $28.95 per hour
Pay based on experience
