Description:
Mercy Fitzgerald Hospital, a member of Trinity Health, is hiring a pharmacist as part of an expansion of night shift/emergency services! We are expanding our night shift coverage to include a decentralized pharmacist. This position is for pharmacists with experience in hospital pharmacy who are able to work directly with patients for medication histories, counseling, and responding to emergencies within the hospital.
Shift: Full Time, 24hrs/wk. 12 hour shifts 8p - 8a. Weekend and Holiday requirements per department needs.
Job Description Summary:
- Appropriately assesses patient data to identify medication-related problems through profile reviews, pharmacy consults and interdisciplinary rounding.
- Participates in medical emergencies and other formal alerts such as trauma, stroke, sepsis and STEMI as required.
- Reviews and interprets providers orders to assure that the drug, dosage form, and dose is appropriate for the patient's age, weight, and comorbidities; checks for drug incompatibilities and allergies, and takes appropriate action when necessary; updates medication history for all admitted patients.
- Consults with providers, nurses and other healthcare professionals involved in patient care regarding pharmacotherapy optimization to achieve optimal patient outcomes.
- Provides accurate and timely responses to drug information questions and medication use policies for safe and appropriate medication use.
- Documents patient care activities including therapy plan, monitoring, and follow-up in electronic medical record per guidelines.
Requirements:
- Pharmacy degree (Pharm.D.) from an ACPE approved college of pharmacy.
- Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years hospital experience.
- Registered pharmacist in Pennsylvania at time of hire.
- Preferred experience with Pyxis MedStation, EPIC.
- BLS and ACLS certification before completion of orientation (provided if cannot provide upon hire).
