What You’ll Do

• Provide individualized physical therapy services to residents
• Develop treatment plans to improve mobility, strength, and independence
• Collaborate with interdisciplinary team members to ensure holistic care
• Document resident progress in compliance with state and federal guidelines
• Educate residents and families on safe mobility and home programs

Qualifications

• Current California Physical Therapist (PT) license required
• Strong clinical skills and passion for patient-centered care
• Excellent communication and teamwork abilities
• SNF or post-acute experience preferred, but not required (mentorship available)
